Terms & Conditions - Physiotherapy

AGREEMENT

Notification of Terms and Conditions

 

Our commitment to you/your client

Please ensure that you read this agreement carefully as not only does it form the legal relationship between us, it also provides useful information. When you sign this agreement (or if you fail to do so) OR [If you are booking and paying online, our acceptance of your order will take place when we email you to accept it, at which point] this document will constitute a legally binding agreement between us.

You will always be assessed by a qualified physiotherapist.

Our physiotherapists are all registered with the Health and Care Professions Council and Chartered Society of Physiotherapists and will examine and treat you in line with the Chartered Society of Physiotherapy's code of practice. After examination the physiotherapist, will explain her/his assessment of the presenting concerns and propose treatment if appropriate, explaining what is involved, including any risks. You may decline any of the treatments proposed, without prejudice.

 

Your consultation and treatment will be on a one-to-one basis (unless otherwise agreed) and in the privacy of the treatment room/cubicle or in a chosen domiciliary setting.

 

Your appointment/s will be scheduled according to your needs and to the best of our ability. 

 

Anyone may book an appointment. This should be done on line or by telephone

 

Letters & Reports

Requested reports and/or letters are charged in respect of the total time taken to prepare them. They are based on the standard therapy costs.

 

Telephone conversations over 15 minutes will be charged and are again based on the above standard therapy costs. Administration charges are billed at £25.00 per hour.

 

Private Health Insurance Claims

Physiotherapy and Pilates is not registered with insurance companies. 

Please note that the ultimate responsibility for settling your account lies with you and you will be required to meet any shortfall arising from any claims. 

 

Cancellation Policy

Physiotherapy and Pilates requires 48 hours-notice of any cancellation.

 

If you need to cancel / re-arrange any appointments, please give 48hrs notice. The way you can contact us to advise us of a cancellation are by telephone to 07740714804 or by email to physiospecialist.t.j@gmail.com

 

 

We are aware that due to the nature of our service some individuals will have to cancel with short notice, however we must treat all clients the same and with less than 48-hours-notice we are unlikely to fill that appointment slot. Therefore, failure to provide 48 hours-notice of a cancellation for any reason will incur a cancellation charge which is the full cost of consultation charges at our clinic or domiciliary visits.

 

If there are a significant number of cancellations made on your account, we will contact you to discuss and agree a solution.

Clinic Appointments

If you are more than 15 minutes late for your scheduled appointment, it will be at the discretion of the therapist if you can be treated or not.

 

Payment Terrms

A deposit for initial consultation must be made at time of booking if booking on line. Payment for all other treatments is required at time of treatment. 

 

Insurance & Regulation

We confirm that Physiotherapy and Pilates hold the appropriate professional indemnity insurance in respect of your treatment. We confirm that our Physiotherapists are regulated by the Health and Care Professions Council, and are members of the Chartered Society of Physiotherapists.

 

DATA PROTECTION

 

We comply with data protection laws in relation to your personal data. Please see our Privacy Notice for details of how we use your personal data.

Consent

Clients are required to consent to assessment and treatment. They are expected to respect the clinician during the treatment sessions. Aggressive and/or abusive behaviour will not be tolerated. Additional permission will be required for audio / video recording and for the circulation of reports to other relevant professionals.

 

Withdrawal

The client is free to withdraw from therapy at any time without stating a reason provided that 24hrs notice is given for any booked appointments.

 

Termination of Therapy

Physiotherapy and Pilates reserves the right to terminate therapy at any time.

 

Physiotherapy and Pilates Ltd reserves the right to amend these terms and conditions from time to time.
